Home Care Field Supervisor – Registered Nurse (RN) Full-Time | Competitive Pay | Meaningful Work in Your Community We are seeking a compassionate, dedicated, and highly skilled Registered Nurse (RN) to join our home care team as a Home Care Field Supervisor. This role is essential in ensuring our clients receive exceptional, safe, and consistent care in the comfort of their homes. As a Field Supervisor, you will provide clinical oversight, support caregivers in the field, and ensure all services meet state regulations and agency standards. Key Responsibilities * Conduct in-home supervisory visits with clients to assess care quality, safety, and client satisfaction. * Perform initial assessments, re-assessments, and ongoing evaluations of client conditions and needs. * Develop, update, and implement individualized care plans according to state requirements and agency guidelines. * Train, mentor, and support field staff—including HHAs, CNAs, and other caregivers—to ensure competency and compliance. * Provide clinical guidance and act as a resource for caregivers regarding client needs, care techniques, and best practices. * Monitor documentation for accuracy, completeness, and compliance with state regulations. * Maintain timely communication with office staff, clients, family members, and care teams. * Participate in on-call rotation as needed to support the agency and ensure continuity of care. * Identify concerns in the field and take appropriate corrective actions to maintain high standards of service. * Ensure all care is delivered in accordance with company policies, licensing standards, and ethical practices. Qualifications * Must hold an active, unrestricted Registered Nurse (RN) license. * Minimum 1 year of nursing experience required (home care experience preferred). * Strong understanding of home care regulations, care plans, and assessment procedures. * Excellent communication, leadership, and interpersonal skills. * Ability to work independently, manage time effectively, and handle multiple client needs. * Compassionate, patient-centered mindset with a commitment to high-quality care. * Valid driver’s license and reliable transportation for field visits.
